Is your patella malaligned? Do you get anterior knee pain? It may be that your knee cap does not quite glide centrally in the trochlear groove as you bend and straighten your knee. This may be improved by releasing the structures on the outer side of your knee called the lateral retinaculum and lengthening the ITB. (Lippincott, Williams and Wilkins, 2003) Stengthening the medial quadriceps namely the VMO will improve symptoms. Other muscles such as the deep calf/ soleus may be at fault too. If you experience significant pain, the management may need to be careful initially as patellofemoral symptoms can easily be aggravated by certain strengthening exercises.
